YOU CHECK-IN INTO THE 

CARE FACILITY

At an agreed-upon time, you’ll meet your Care Coordinator and Client Recovery Advisors, receive an overview of the IPAT program, and identify what should accompany you in your recovery journey. We’ll work with you to make sure our “treatment space,” is fully tailored to your treatment needs.

WE CREATE YOUR CUSTOM 

TREATMENT PLAN

Your Care Coordinator—a Nurse/Social Worker who develops and maintains your IPAT treatment plan—will help customize your program, schedule your psychiatric evaluation, and identify any necessary outside providers (Primary Care Provider, Therapist, or Medical Specialists). 

GOAL-SETTING & COMMUNITY RESOURCES

Your Client Recovery Advisors (CRAs)—the peer support members of your IPAT team—will meet with you often, connecting you with community recovery resources and putting your Recovery Wellness Plan into place.

Your CRAs will take you through a series of Intervention Guidelines that directly apply to what you’re struggling with. 

BUILDING FAMILY SUPPORTS

A Family Education Facilitator will meet with your family to support them in your recovery, educate them about the recovery process, encourage self-care, and connect the family to recovery resources. When the time is right, Addiction-Care will connect you with a Family Systems Therapy Provider to work with you and your loved ones in the healing process.

IN-TREATMENT & POST-TREATMENT SUPPORT

Throughout the entire treatment process there will be many team meetings to ensure continued alignment with the client, the care team, the family, and their goals. Aftercare options include halfway houses, life skills, support groups, and one-on-one therapy
